How to Apply - for volunteer clinicians:

Disaster privilege application for COVID-19 operations at UCSF Medical Center (San Francisco locations: Langley Porter, Mount Zion, Mission Bay, Parnassus)

Step 1 - (for credentialing administrative contacts):

UCSF Credentialing Contacts - if your service Chief or Chair has identified clinicians they wish to bring on for COVID-19 operations, please have the Chief directly forward these names to the Office of Medical Affairs and Governance:

 

Step 1 - (If you are volunteering unprompted):

Receive notice of intent from COVID Labor Pool contacts

Step 2:

Complete the Disaster Privilege Application Form
  1. Our office may request brief comments on any settled or open malpractice cases
  2. Confirmation of disaster privileges will arrive via email. 

Step 3

Report to your UCSF clinical supervisor/liasion or to your clinical location with your:
  1. Valid and current Hospital ID, and
  2. Valid and current government ID.
The scope and duration of these privileges are dependant on general and specialty training and/or licensure. They will remain in effect until the crisis has been declared over by the Medical Center HICS.
