Application Process

Navigating a large, complex environment with many relationships can feel overwhelming. This is a list of detailed responsibilities that are need to be completed for applications to be approved by the UCSF Credentials Committee and the Committee on Inter-Disciplinary Practice.

Applicant Responsibilities

8 to 6 Months Before Start 


Apply for a California license (as early as possible)


Ensure federal DEA license lists a California address


Get current BLS/PALS/ACLS/NRP certification -  if required for your position


For midwives, nurse practitioners and physician assistants: complete a State of California approved controlled substance education course:

Completes background check information link via email from HR

 

 

6 to 2 Months Before Start 


Complete online credentialing application (UC Me) promptly after receipt of our office’s notice.


Complete a signature form for e-signature of electronic credentialing application 


Print, sign, and mail us the UCSF Health Plan Enrollment Application (new providers only)


Review these medical staff documents:


Provide current malpractice insurance certificates of insurance (COI) if applicant is not covered by UC Regents (i.e. UCSF employed/volunteer)

 

 

Prior to Clinical Patient Care


Complete online Learning Center training:


Complete all required Occupational Health Services screenings which may include:

  • PPD test (N-95 fit testing, influenza compliance, etc.)
  • New applicants may need to submit proof of immunization for Measles, Mumps, Rubella, Varicella, Tdap, and Positive Hepatitis B titer

Ensure NPI / NPPES taxonomy code and new UCSF practice/contract addresses are current and up-to-date

Administration Responsibilities

Department/Clinic/Affiliated Group Expectations

Pre-application submission, including the provider’s primary specialty and Chair Recommendation Letter. Include the provider’s future privileges, this helps expedite this process - (This includes submission for temporary/traveling providers.)

Ensure applicant is applying for the appropriate privileges for current role 


Submit MAF to via Peopleconnect and request MyAccess account


Submit ARF for new employees early - including eEPS request


Submit ePrescribe request  (if required)


Monitor provider’s application in UC Me and help department leadership (designated signatory) promptly review/sign files


Provide support when credentialing team encounters unresponsive parties (i.e. references, hospitals, applicants, etc.) 


For clinically integrated partners (CIP/CIN), ensure submission of New Apex User Access form to ADT/Access team. Account Request Form (ARF) is required for new user log-on, and finally request appropriate training classes for new user


 

Office of Medical Affairs and Governance Responsibilities


Receipt/processing of  pre-application


Creation of provider's online profile to generate login and password  for applicant to complete the application


Primary source verify completed application details, and credential to applicable CMS, TJC (JCAHO) AAAHC, and/or NCQA standards


Identify flagged items for the departmental clinical leadership to review/provide comments


Route the finalized files to the department for clinical leadership review, and recommend appointment status/timeframe to the Credentials Committee or Committee for Interdisciplinary Practice (CIDP)


Monitor ongoing sanctions, exclusion lists, expiring licenses, certifications, etc.

UCSF Clinical Leadership Roles


Timely review, recommendation, and comments (if comments are required) for an applicant’s credentialing file


Assess (and when appropriate discuss) any relevant flags or concerns uncovered by the credentialing process


Review the file as presented and to consider signing off on the credentialing that was completed.  Committee members look at the complete file and confirm whether this person will be safe and competent to see patients and join our physician network.
